This program serves geriatric resident in community settings. Following an assessment   process, necessary services to promote and/or maintain independent living are then coordinated, including: monitoring, advocacy, referrals, crisis intervention, in service training, community education, and individual/group/family therapy.                                    

A Service is available to link clients to a work service program, to assess employment skills, and provide the needed support to obtain and maintain employment in the community.

Assistance in accessing, linking and monitoring services is offered through a person-centered process to support consumers in the community. Assessment, follow-up, advocacy, referral and coordination of services/supports are also available to the consumer.

Specialized rehabilitative and habilitative training in self-care, communication, daily living skills, mobility and other skills necessary for consumer living. Services are provided in an environment that promotes community inclusion, leading to increased independence.
