What are Case Management Services?
A Case Manager supports individuals in gaining access to medical, social, educational,
behavioral health, vocational or other services individuals may need to maximize
their independence. Case Management is generally provided within the home, in a
community or vocational setting, and varies in intensity based on the needs of the
individual being served.
Who can benefit from Case Management Services?
An individual with a mental illness may be eligible for Case Management Services.
Priority in provision of services is given to individuals with the most severe need
and to those in urgent or emergent situations.
How do Case Management Services help an individual?
A Case Manager supports individuals in more fully participating in their community.
A person centered treatment approach is used to help identify an individual’s strengths,
dreams, goals and desires. The Case Manager may help to access the most appropriate
living environment; encourage an individual to achieve a higher level of independence;
assist with coordination of assessment, monitoring, advocacy, crisis intervention
and other services, or refer an individual for the appropriate services required.
