What are Geriatric Outreach Services?

Geriatric Outreach Services provide support to individuals in gaining access to medical, social, educational, behavioral health, vocational or other services individuals may need to maximize their independence in the least restrictive environment possible. Geriatric Outreach Services are generally provided in homes, or community settings and vary in intensity based on the needs of the individual being served.

Who can benefit from Geriatric Outreach Services?

An older adult with a mental illness may be eligible for Geriatric Outreach Services. Priority in provision of services is given to individuals with the most severe need and to those in urgent or emergency situations

How do Geriatric Outreach Services help an individual?

The Geriatric Outreach Program addresses various problematic situations that confront older adults living independently in the community. A person centered treatment approach is used to help identify an individual’s strengths, dreams, goals, and desires. A Geriatric Outreach Coordinator may help to access the most appropriate living environment; encourage an individual to achieve a higher level of independence; assist with the coordination of assessment, monitoring, advocacy, and crisis intervention services or refer an individual to the appropriate services required.
