Home-Based Services
What are Home-Based Services?
Home-Bases Services provide intensive services to individuals and families with
multiple service needs who require access to a comprehensive array of mental services.
The family unit is the focus of the treatment. Intensive in home treatment is provided
for families including therapy, Case Management, parenting education, and family
skill building. Intensity of services varies based on the needs of the individual
or family.
Who can benefit from Home-Based Services?
A family with a child under 18 years of age, with emotional difficulties may be
eligible for Home-Based Services. Priority in provision of services is given to
individuals with the most severe need and to those in urgent or emergent situations.
How do Home-Based Services help an individual?
A Home-Based Services Coordinator supports children and their families in more fully
participating in their community. A person centered treatment approach is used to
help identify an individual’s strengths, dreams, goals and desires. Services promote
normal development, promote healthy family functioning, support and preserve families,
reunite families that have been separated, and reduce the usage of, or shorten the
length of stay in, psychiatric hospitals and other substitute care settings.
How can you access Home-Based Services?
An individual may request services directly by another service agency or professional.
