Intensive Case Management Services
What are Intensive Case Management Services?
Treatment is provided by a team of professionals, offering an array of clinical,
medical or rehabilitative services to support an individual in living independently
in the community or provide support in the transition to independent living from
a more restrictive setting. Intensive Case Management services are generally provided
in homes, community or vocational settings and vary in intensity based on the needs
of the individual being served.
Who can benefit from Intensive Case Management Services?
An individual with a serious mental illness may be eligible for Intensive Case Management
Services. Priority in provision of services is given to individuals with the most
severe need and to those in urgent or emergency situations
How do Intensive Case Management Services help an individual?
An Intensive Case Management Services Team supports individuals in more fully participating
in their community. A person centered treatment approach is used to help identify
an individual’s strengths, dreams, goals and desires. The Intensive Case Management
Services Tem May help to access the most appropriate living environment; encourage
an individual to achieve a higher level of independence; provide support with medication
administration; assist with the coordination of assessment, monitoring, advocacy,
crisis intervention, skill building and other services; or refer an individual for
the appropriate services required.
