| Q: WHAT IS
PERSON CENTERED PLANNING OR PCP? A: This is a way for you (and your family, parents, or guardian) to be involved in planning your treatment, to decide what help you need, and to find other services based on treatment available. Q: HOW DOES PCP WORK? A: PCP happens in four parts: preplanning activities; planning for your future; arranging for the help you need; and giving your opinion so the plan meets your needs. |
Q: HOW WILL
PCP HELP ME BE SAFE? A: PCP makes sure your health and safety issues are considered and addressed in your treatment plan. Q: DOES PCP ALLOW FOR MY FRIENDS, MY FAMILY, OR MY WORKERS TO BE INVOLVED IN MY TREATMENT PLANNING? A: Yes. PCP is designed to get input from the people who care about you. You may invite anyone you wish to help make your plan. PCP works because people important in your life are there to help make sure your dreams, wants, and needs are included in you plan. |
CHECKLIST FOR PCP: __I invited people important to me to my meeting. __I will be responsible to say what I want or need. __My wants or needs were put into my plan. __We talked about my health and safety. __Changes in my plan were made only after I discussed them with my worker. __I had my PCP meeting at a time, date and place that I agreed to. __We talked about other community resources and supports that might be helpful and available to me. __I will be responsible for the choices I make. |
