Supports Coordination
What is Supports Coordination?
Supports Coordination provides assistance to individuals with developmental disabilities
in gaining access to medical, social, educational, behavioral health, vocational
or other services individuals may need to maintain or improve their independence
and quality of life. Supports Coordination is generally provided within the home,
community or vocational setting, and varies in intensity based on the needs, desires
and preferences of the individual being served. The essential elements of Supports
Coordination include, but are not limited to assessment, linking, and coordinating
services as well as follow-up and monitoring service effectiveness.
Who can benefit from Supports Coordination?
Individuals with a developmental disability may be eligible for Supports Coordination.
Priority in provision of services is given to individuals with the most severe need
and to those in urgent or emergent situations.
How does Supports Coordination help an individual?
A Supports Coordinator supports individuals to more fully participate in their community.
A person centered approach is used to identify an individual’s strengths, dreams,
goals, desires and preferences. A person-centered planning meeting is held to develop
a plan which will be used to assist the individual in building skills essential
to maintaining or improving their independence and quality of life.
