Background: As you may know each
Community Mental Health organization (CMH) throughout the state of Michigan must submit in
Application for Participation (AFP) in order to retain its role as the manager of
Medicaid-funded mental health care in their community. This CMH is submitting this
application as part of four CMH (six county) affiliation that covers Gratiot, Ionia,
Newaygo, Clinton, Eaton, and Ingham Counties.
Stakeholder Review: This CMH, and its affiliate partners,
see great value in the review, of the draft Application for
Participation (AFP), but stakeholders drawn from the communities of the four Affiliate
CMHs. This review is key to ensuring that the Application outlines a course of action
that:
- Ensures consumer choice and empowerment
- Reflects local needs
- Meets the requirements, as established by the
Michigan Department of Community Health
- Reflects leading edge clinical and supports
provision approaches and industry standards relative to a wide range of administrative and
care management functions
- Ensures that the public/common good is
promoted
Invitation to Review and Comment:
We are inviting your involvement in the review of this CMHs AFP, according to
the following timelines. This process, and its timelines, were developed by stakeholders. Your comments, and those of many others
(CMHs Advisory Councils, etc.) will be used to guide the development of the AFP and
the work of this CMH after the AFP submission.

and Guidance to Stakeholders in Reviewing Narrative:
The AFP narrative that is being provided, by this CMH, for review by stakeholders, is the narrative background to
the check-off format required by the Michigan Department of Community Health.
This narrative provides the reader with the detail necessary for readers to understand the direction of this CMH and its
affiliate partners relative to the management of Medicaid mental health services in this
community.
A number of stakeholders, in light of the length of the document, have recommended that
stakeholders who wish to review and comment on this
document focus their attention on those areas of greatest interest to them.
Stakeholders should use the topics list, below, to find the number of the areas of
interest to them and then use this number to find the corresponding section of the
narrative.
